Great attempt I can see a lot of likenesses between this piece and the forge world one, little details like the grooves in the teeth really help too.

He looks like a jolly old man smoking a pipe, perfectly fitting the 'Grandfather Nurgle' theme, he also reminds me a bit of Sir Daniel Fortesque from the MediEvil video game,with the angle of the cheekbones & eyeballs. The skull-cheeks complement the flabby neck very well, they look disturbingly wrong, but not anatomically incorrect. I do like most muchly.
